Research interests
My research focuses on men’s sexual and reproductive health, including erectile dysfunction, Peyronie’s disease, penile fibrosis, and post-prostate cancer sexual dysfunction. I study male infertility, varicocele, semen quality, DNA fragmentation, and hormonal optimisation. Additional interests include penile cancer, testicular oncology, priapism, penile trauma, and reconstructive urology. My translational work covers stem-cell therapy, nerve regeneration, pelvic neurophysiology, and fibrosis modulation. I also research testosterone deficiency, premature ejaculation, psychosexual health, and the role of digital health, AI, and real-world evidence in improving men’s health pathways.

Publications
Fabio Castiglione has authored over 150 peer-reviewed publications in andrology, sexual medicine, male infertility, penile cancer, reconstructive urology, regenerative medicine, and pelvic neurophysiology. His work includes original research, systematic reviews, consensus statements, translational science, and multicentre clinical studies across Europe.
His clinical research covers:
� Erectile dysfunction, Peyronie�s disease and penile fibrosis � including landmark ESSM consensus statements, stem-cell therapies, stromal vascular fraction, and cavernous nerve injury models.
� Penile cancer and testicular oncology � multiple studies with EAU Young Academic Urologists on penile-sparing surgery, lymphadenectomy outcomes, sentinel node biopsy, and prognostic factors.
� Priapism, penile trauma and genital reconstructive surgery � prospective cohorts, Doppler-based analyses, and novel surgical techniques.
� Male infertility and varicocele � systematic reviews, meta-analyses, outcomes after varicocele treatment, DNA fragmentation, and fertility optimisation.
� Urethral stricture and reconstructive urology � international multicentre studies on urethroplasty, grafting materials, fixation techniques, and urethroscopic predictors.
� Sexual medicine epidemiology � ED in young men, testosterone pathways, premature ejaculation, pharmacological treatments, and sexual function after prostate cancer treatment.
� Regenerative medicine and basic science � pioneering work on stem cells, nerve regeneration, inflammation pathways, urethral fibrosis, ureteral physiology, and bladder innervation.
He has produced numerous systematic reviews and meta-analyses, including high-impact work on ED therapies, penile enhancement, Peyronie�s grafting, varicocele management, premature ejaculation, and ischemic priapism.
He is a co-author of multiple international consensus statements, including:
� ESSM consensus on animal models for Peyronie�s disease
� ESSM/EAU statements on cavernous nerve injury models
� eUROGEN/EAU consensus on penile cancer management during COVID-19
His translational science includes collaborations with Johns Hopkins, KU Leuven, UCLH, King�s College Hospital, and EAU research networks, with work published in European Urology, Eur Urol Focus, Sexual Medicine, IJIR, J Urol, BJOG, BJU Int, Nat Rev Urol, World J Urol, and others.
