About me

If you're considering therapy, you may be dealing with anxiety, low mood, relationship strain or stress that is showing up physically (for example IBS-type symptoms, headaches, chest tightness, dizziness or fatigue). You may be functioning on the surface, but privately stuck in overthinking, self-criticism, people-pleasing, emotional shutdown or a persistent sense of pressure and unease.

I'm Dr Zach Vogiatzis, a Chartered Counselling Psychologist (HCPC registered) at The Psychology Clinics in Central London (W1W) and online. I specialise in anxiety/stress and low mood, burnout/high-pressure functioning, relationship and attachment difficulties, and stress-related and medically unexplained (persistent) physical symptoms.

A particular focus of my work is stress-related and medically unexplained (persistent) physical symptoms (PPS), especially when medical tests have been reassuring but symptoms persist.

Areas I can help with

I commonly work with adults experiencing:

Anxiety, stress and panic (including health anxiety)
 Depression/low mood
 OCD difficulties
 Trauma-related symptoms and PTSD
Work stress, burnout, perfectionism and imposter feelings
Relationship difficulties (repeating patterns, overcontrol, conflict avoidance, emotional withdrawal/shutdown, people-pleasing)
 Bereavement and complicated grief
Stress during pregnancy, postnatal adjustment and major life transitions

Examples of stress-related and medically unexplained (persistent) physical symptoms (PPS), include (but are not limited to):

IBS-type symptoms and other gastrointestinal distress
Persistent muscle, joint, or back pain
Dizziness or light-headedness, faintness, tinnitus, or visual disturbance
Functional presentations (for example non-organic/cardiac chest pain or non-epileptic seizures
 Tension headaches and migraine
 Psychogenic movement symptoms/disorders
Symptoms that continue despite medical reassurance
 Fibromyalgia

How I work

Therapy with me is active and collaborative. We identify the emotional and relational patterns that keep difficulties going and map your cycle in real time (sensations, thoughts, feelings, behaviours and context). This is particularly helpful where stress-related or medically unexplained persistent physical symptoms (PPS) are present.

Together, we reduce symptom fear, avoidance and reassurance loops, strengthen emotional capacity (feeling without flooding or shutting down), and work through the underlying feelings driving anxiety and stuck patterns. We also shift repeating relational dynamics such as people-pleasing, overcontrol and emotional withdrawal, so changes hold in everyday life and relationships.

In my work, I integrate evidence-based approaches such as ISTDP and DIT (relational/psychodynamic), CBT and where appropriate, EMDR, adapting the pace and focus to you.

What you can expect

Clients often tell me they value therapy that is both human and purposeful. You can expect an initial phase to understand your difficulties and agree goals, followed by focused work that stays grounded in day-to-day change.

We review progress regularly and adjust the work as needed. If you are struggling with persistent physical symptoms (PPS), we will map your symptom cycle, reduce fear and tension loops, and build steadier regulation over time alongside appropriate medical care.

Getting started

An initial appointment helps us understand what's bringing you to therapy, agree priorities and decide whether my approach is the right fit. If we proceed, we'll set clear aims for the work and a plan that matches your needs.
